The Encounter An encounter is defined as 'a face-to-face contact' between a health care professional and an eligible beneficiary. Each time a person/patient/client is seen in a medical office and receives medical attention or other forms of health care services (including extensive consultation over the phone or at home if medically necessary) it is referred to as an encounter.

The Claim While most doctors would rather collect payment from patients right after the office visit, or hospital stay, it is unrealistic! The majority of their patients have health insurance. To get reimbursed, fees for medical services must be processed by filing a claim with the patient's health insurance providers, managed care organization, or government entities.
Filing a claim for reimbursement involves examining patient records, identifying diagnoses, often based on the lab procedures, signs and symptoms and procedures, then selecting appropriate codes for the services performed from different coding systems.
